  1. Complete the online “scholarship donor information" form and pay via GiveMN.org

    OR

    Download, print and complete the “scholarship donor” information form; mail form and check to:

    Eagan Foundation, Inc.
    c/o Dakota County Regional Chamber of Commerce
    P.O. Box 211192
    Eagan, MN 55121

    Download the scholarship donor information form (right-click the link, select "Save Target As", save to your computer)

  1. Scholarship Donors will be invoiced accordingly.
Friend of EFI United Way Designation: Write the Eagan Foundation name and address on your pledge form or gift-designation card and United Way will see that your contribution is sent to us:
